In a mixing bowl, combine lean ground beef, ketchup, Worcestershire sauce, onion powder, kosher salt, and black pepper. Mix gently and shape into 4 patties, slightly larger in size than the bread slices.

Melt 2 tablespoons unsalted butter in a cast-iron skillet over medium heat. Add the beef patties and cook 4 to 5 minutes per side until browned and cooked through. Remove from skillet and set aside.

Wipe the skillet clean and add the remaining unsalted butter. Arrange 4 slices of rye bread on a work surface. Place one cooked patty, a generous portion of caramelized onions, and 2 slices of Swiss cheese on each. Top with the remaining slices of bread.
